To complete your identity verification, please reply and attach 1 of the following documents:
Bank correspondence (e.g. statement, credit card bill, proof of account)
Utility bill (e.g. landline, electricity, water, internet, or similar)
Government issued document (e.g. tax bill, pension statement)
 
It is important this:
is dated within the last 3 months
clearly shows your name and residential address as submitted in your application
Acceptable POA Document formats: PDF, Scan of paper based document, photo of a paper based document.  NOTE: a photo of a computer screen is NOT acceptable
